Although publicly owned, day-to-day operation is controlled by NHS managers who, although paid by the government through the NHS, are not directly affiliated to the government but, rather, they are affiliated with the NHS so the size of the operational bureaucracy required for operation from the governments perspective would be close to, if not equal to, the size of the bureaucracy required to distinguish the cost of payments to private companies in regards almost 200m people.
